DOL Audit Notice Request List

The Employee Benefits Security Administration (EBSA) of the U.S. Department of Labor (DOL) is responsible for the administration and enforcement of Title I of the Employee Retirement Income Security Act of 1974 (ERISA) . ERISA establishes standards governing the operation of employee benefit plans and the conduct of fiduciaries with respect to such plans. Section 504 of ERISA vests investigative authority over employee benefit plans with the Secretary of Labor.

When an employer’s health and welfare benefit plans (Plans) have been selected for a limited review, typically the investigation will consist primarily of an examination of Plan records for a specified period, and an interview with Plan fiduciaries.

In order to facilitate the investigation, EBSA provides employers with a list of documents that should be sent to its office for inspection and retention within a specified time frame.
